Please contact [email protected] to receive support in requesting a sick leave or a workplace accommodation because of a disability or medical condition.
While on a sick leave, has your physician been contacted directly by a representative from the York University Health, Safety & Employee Well-Being Office (EWB) with requests for your personal medical information without your knowledge, consent and/or permission? If so, please contact [email protected].
YUFA has become aware that the Employer has now made it mandatory for members who require a sick leave to sign the Medical Certificate form which includes a statement of permission for the York University Health, Safety & Employee Well-Being Office (EWB) to correspond directly with your physician about your medical condition. YUFA's position is that members should control the sharing of their own confidential medical information with the EWB Office.
YUFA has raised objections with the Employer about this new practice and is exploring next steps. In the meantime, the Association recommends that members who submit medical certificate forms to the EWB should write a follow-up email to the EWB indicating that you do not consent to the EWB Office directly exchanging information with, or releasing information to, your licensed medical practitioner about your medical condition.
